A Practice can carry out its commitment to provide the highest quality medical care to you, the patient, only when a mutual understanding of expectations exists between you and the Practice. Our requirement for maintaining your account in good standing is:
We understand there are circumstances that may warrant an extended payment plan. To avoid any misunderstandings, we invite early discussion of financial problems. We have payment plans available that require additional financial information.
If we are a non-participating provider in your health plan and are submitting claims to your insurance company, you may continue to receive a statement until your account is paid in full. If we are participating providers with your health plan, you will only receive a statement from us for the deductible, coinsurance and non-covered services once your health plan has processed your claim. Statements are sent on a monthly basis. Your initial statement will be itemized. Your second statement will indicate a balance forward. It is important to save your initial itemized statement as a point of reference for future bills. Click here to view a sample billing statement.
