Billing Information

Generally speaking, the payment policies of the SBU Physicians-affiliated practices are outlined below. However, individual practices may adopt different payment policies from time to time.

A Practice can carry out its commitment to provide the highest quality medical care to you, the patient, only when a mutual understanding of expectations exists between you and the Practice. Our requirement for maintaining your account in good standing is:

  1. Copayments are expected to be paid at the time of service.
  2. Unless the provider is participating with your health plan or a special arrangement has been made, all charges are due and payable at the time of service.
  3. If you are on a payment schedule for a previously unpaid balance we may ask you to prepay for current services.
  4. Depending on your insurance coverage, you are responsible for the timely resolution of your account balance. If your claims are delayed and you receive notification, please contact your insurance company to expedite processing.
  5. If your insurance coverage requires referrals, please confirm with your Primary Care Physician (PCP) that one has been issued to the specialist.
When you send in a payment by personal check, please include the top portion of your billing statement and indicate your medical record number on your check. This helps us expedite your payment. For your added convenience, we accept VISA, MasterCard, and Discover.

We understand there are circumstances that may warrant an extended payment plan. To avoid any misunderstandings, we invite early discussion of financial problems. We have payment plans available that require additional financial information.

If we are a non-participating provider in your health plan and are submitting claims to your insurance company, you may continue to receive a statement until your account is paid in full. If we are participating providers with your health plan, you will only receive a statement from us for the deductible, coinsurance and non-covered services once your health plan has processed your claim. Statements are sent on a monthly basis. Your initial statement will be itemized. Your second statement will indicate a balance forward. It is important to save your initial itemized statement as a point of reference for future bills.

Our Customer Service phone line, is available Monday through Friday from 8:00 am - 4:30 pm. Please be advised that our peak hours are between 11:30 am to 2:30 pm.
